TERMS AND CONDITION:
● All included services rendered in-office.
● Appointment required.
● Member may bank up to 1 service to use in another month.
● Upgrades may be added at time of service for an additional fee.
● Additional units will be available at the time of service for a discounted rate.
● We reserve the right to refuse membership to anyone at our discretion.
● We reserve the right to change the terms of this membership at any time without notice.
● Member discounts are not combinable with other promotional discounts.
● Reward points may not be combinable with member discounts.

CANCELLATION TERMS:
● A 6-month minimum commitment is required. Your membership will automatically renew month-to-month.
● Cancel in writing, we require a 30-day Notice to Cancel
● This membership may be canceled at any time by Le Petit Spa at its sole discretion.
● The member may cancel the membership at any time after the initial 6-month term by providing written notice no less than 30 days prior to the requested cancellation date. If written notice is not received within this timeframe, the member will be responsible for the following month's payment. Written notice may be submitted via a letter or an email to priti@lepetitspacharlotte.com, including the member’s full name and membership ID (if applicable).
● All memberships are non-refundable, regardless of whether they have been used or not. If the client no-shows or cancels late, the $99 credit will be applied toward the cancellation fee. Since we charge 100% according to our policy, the remaining amount will be charged to the client’s credit card, excluding the $99 credit.

WAIVER AND RELEASE OF LIABILITY
To the fullest extent permitted by law, Le Petit Spa is not responsible for any injury or loss of property suffered by me while receiving Spa treatment(s), participating in Spa activities, or while on the premises of Le Petit Spa, for any reason whatsoever, whether resulting from the negligence of Le Petit Spa, its agents, employees, independent contractors, representatives, or guests, members, customers, or any other person or entity.
I acknowledge and understand that there are inherent risks associated with Spa treatments and activities.
